| My wife name Mrs.Rekha was sudenly admitted in DHANWANTARI HOSPITAL for deliveri on20/07/2007 to 3/08/2007 as per Doctor's advice,at that time during this period I PAID a hospital expens amounting of Rs.29200/- Affter this I made application for Reimburshment on 22/11/2007 then I get reply from E.S.I.C Aundh Hospital PUNE for fullfill the few requirment vide there letter no P.K-1233/08/DATED 19/03/2008& I was submitted the same to your Aundh Hospital on 29/08/2008 (2565 tartudi babat) from AUG -2008 TO TILL TO-TODATE I didnot get any information or reply from your office ,I frquently visited to your E.S.I.C HOSPITAL ,MOHANNAGR ,CHINCHWAD,PUNE for status of claim but they say that the claim was send MUMBAI OFFICE for approval but there was not geting the referance when the claim was send to MUMBAI OFFICE all this ,it gets TWO YEARS PERIOD when I will get my claim amount THEREFOR,I AGAIN REQUEST YOU TO SETTEL MY CLAIM AS EARLY AS POSSIBELE ETITHER GUIDE ME HOW TO SETTEL MY CLAIM .AWITING FOR YOUR FAVOURIBAL REPLY YOURS K.M.CHAVAN 9921765553 | Ins. | Non-recival of Pahchan card due to change of employer. Dear sirs, Iwas staff of Delhi Press Patra Prakashan Pte Ltd, Kolkata Branch, on 31 July,2010. when I and my family was photo-shot for Phachan card at 9A/9B, Swarnamotee Road, Kolkata-700009 ESIC office. At present I am staff of RICE Group, Kolkata, ESIC reg. no.-40000 3825 0000 1329. Your above ESI office refuses to deliver me my card to me personally. Both my present employer and ex-empolyeer is not willing to send somebody to your above ESI office to bring said card for a single person i.e. me.
